I consent and agree that the Toledo-Lucas County Port Authority, its employees, and agents have the right to take photographs, videotape, or digital recordings of me during the AccessTOL event and to use my name and/or likeness and/or voice/photograph for promotion and/or advertising purposes in any manner and any medium, including without limitation, radio broadcasts, newspapers, television, videotape, distribution over the internet and picture data storage which the Port Authority may deem appropriate and hereby release the Port Authority, its employees, and agents from all claims, expense and liability relating to said photographs, videotape, or digital recordings or the use thereof. I further hereby release the photographer and his/her legal representatives and assigns from all claims and liability relating to said photographs. I understand and agree that I will not be compensated for the use of my photographs, videotape, and/or digital recordings. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used. If I am filling out this form as a parent/legal guardian, I give my consent on their behalf.
